The Return of Tourniquets
نویسنده
چکیده
Uncontrolled hemorrhage from isolated penetrating extremity wounds can result in 100% mortality, but significant controversy still surrounds tourniquet use in civilian prehospital care. It’s been proposed that 10% of combat deaths in the Vietnam War resulted from uncontrolled hemorrhage from extremity wounds; many of the deaths were due solely to ineffective field hemorrhage control methods.1 Multiple U.S. and foreign military reports confirm the importance of tourniquets in controlling exsanguination on the battlefield.2,3 As a result, the use of tourniquets is now actively promoted in the U.S. military, with all service personnel carrying a tourniquet with their gear.2

Findings in Mitral Valve Prolapse*
Changes in left ventricular volume appear to be important in changing the timing and duration of mitral valve prolapse. In this study, simultaneous phonocardiograms and echocardiograms were recorded in 16 patients with late systolic mitral valve prolapse before and after the inflation of congesting cuffs (tourniquets) on the upper and lower extremities. The tourniquet application, known to redu...
متن کاملA study of microbial colonisation of orthopaedic tourniquets.
INTRODUCTION Tourniquets are employed widely in orthopaedic surgery. The use of the same tourniquet on a repetitive basis without a standard protocol for cleaning may be a source of cross-infection. This study examines the contamination of the tourniquets in our institution. MATERIALS AND METHODS Agar plates were used to take samples from 20 tourniquets employed in orthopaedic procedures. Fou...
متن کاملTourniquets exposed to the Afghanistan combat environment have decreased efficacy and increased breakage compared to unexposed tourniquets.
We hypothesize that an anecdotally observed increase in tourniquet breakage and decrease in efficacy may be secondary to environmental exposure during military deployment. This was a study comparing efficacy and breakage of 166 Afghanistan-exposed tourniquets to 166 unexposed tourniquets. Afghanistan exposure was defined as tourniquet carriage by field staff in the operational environment for a...
متن کاملReusable venesection tourniquets: a potential source of hospital transmission of multiresistant organisms.
OBJECTIVE To determine the prevalence of multiresistant organism (MRO) colonisation of reusable venesection tourniquets. DESIGN AND SETTING A prospective study in a tertiary hospital to collect and analyse reusable venesection tourniquets for the presence of MROs - methicillin-resistant Staphylococcus aureus (MRSA), vancomycin-resistant enterococci (VRE), and extended-spectrum β-lactamase and...
متن کاملReducing the potential for phlebotomy tourniquets to act as a reservoir for meticillin-resistant Staphylococcus aureus.
The contamination rate of phlebotomy tourniquets with meticillin-resistant Staphylococcus aureus (MRSA) was assessed, and it was determined whether this could be reduced by changes in practice or by the use of a physical barrier. Initially, the tourniquets of both preregistration house officers and phlebotomists were investigated, but as phlebotomists reported significantly more venepunctures d...
